Your provider creates a treatment plan tailored to your condition, your lifestyle, and your goals — including medications, monitoring frequency, lifestyle adjustments, and referral needs.

3. Ongoing Monitoring & Visits

We schedule regular check-ins (typically every 3–6 months) to review your labs, assess your response to treatment, and adjust your plan as needed.

4. Coordination & Referrals
